Overview
Social Worker – Medical Social Worker (MSW) in hospice care, traveling assignment in Helena, MT. This travel - temporary assignment supports patient care in a hospital setting. Start date: 11/15/2026. Contract length: 13 weeks with guaranteed 40 hours per week. Shift: 8-hour days, 8x5. Weekly pay: $2,287–$2,377.

Responsibilities
- Perform biopsychosocial assessments for hospice patients and families; contribute to individualized care planning
- Develop and implement care plans in coordination with an interdisciplinary team; support discharge planning and community resource referrals
- Provide psychosocial support, counseling, and education to patients and families about goals of care, coping strategies, and advance directives
- Document assessments, plans, interventions, and progress in the Epic EMR; maintain accuracy and HIPAA compliance
- Assist with safety, mobility, and functional support; lift up to 50 lbs intermittently as clinically indicated
- Travel to patient locations within the assigned radius; navigate varied road conditions and weather as needed
- Collaborate with the department lead and other team members to ensure comprehensive, patient-centered care

Requirements
- Active driver’s license and access to a reliable vehicle capable of driving in varied conditions (gravel roads, snow, rain, ice)
- Active Basic Life Support (BLS) certification
- Master’s degree in Social Work (MSW); at minimum, working toward Licensed Clinical Social Worker (LCSW)
- Minimum 2 years of clinical/ED/Medical Social Worker experience
- Ability to work in a hospice setting with adult patients; majority of patients are 65 years and older
- Ability to perform lifting up to 50 lbs intermittently
